Note 1: We don't need to know the Computer and Operating System if the "Tape/disk format" you specify is a standard format, such as an IBM Standard Label, Fixed-Block tape format.
Note 2: We don't need to know the Computer make and model if you are converting to a Windows system. Simply state the version of Windows in the "Operating System" field.
Note 3: Massachusetts Data Security Law 201 CMR 17.00 requires that any "Personal Information" on a Massachusetts resident that is sent via common carrier be encrypted, if "technically feasible".
